ABSTRACT

Abstract: introduction: The temporalis muscle plays an essential role in mastication and is actively involved in the mandibular closing movement. It is covered by a fibroelastic fascia that forms its tendon. Tendinitis is a degenerative and inflammatory process, which originates in the tendon-bone junction. Signs and symptoms such as swelling, pain, tenderness on palpation, limitation of movement and mouth opening are frequently associated with other temporomandibular disorders and not with tendinitis as a causal factor. Objective: To describe a clinical case identifying the diagnostic process and management of tendinitis of the temporalis muscle. Case report: A 30-year old male patient who sought treatment after continuous squeezing pain in the zygomatic and bilateral temporal regions with increased pain during mouth opening and mandibular function. The patient referred pain in the insertion region of the tendon of the temporalis muscle. Pain was removed after using anesthesia, consequently confirming the diagnosis of tendinitis of the temporalis muscle. Primary management measures were performed and then peritendinous corticosteroids were administered. The patient did not refer spontaneous or functional pain during check-up. Conclusion: Tendinitis of the temporalis muscle is a common condition, although frequently underdiagnosed. A good differential diagnosis must be performed to avoid confusion with other common conditions such as odontogenic pain, sinusitis, arthralgia, myofascial pain and migraine. Management depends on the type of tendinitis. It usually occurs in conjunction with other types of TMD or facial pain, so it is important to know the different clinical characteristics of pathologies with similar manifestations.


Resumen: introducción: El músculo temporal es fundamental en la masticación, participa activamente en los movimientos de cierre mandibular. Está recubierto por una fascia fibroelástica que conforma su tendón. La tendinitis es un proceso degenerativo e inflamatorio, que se origina en la unión tendón-hueso. Con frecuencia, signos y síntomas como inflamación, dolor a la palpación, limitación del movimiento y apertura, son asociados a otros trastornos temporomandibulares no considerando la tendinitis como un factor causal. Objetivo: Describir un caso clínico identificando el proceso diagnóstico y el manejo de una tendinitis del músculo temporal. Descripción del caso: Paciente sexo masculino de 30 años, acude por dolor opresivo continuo en la región cigomática y temporal bilateral con aumento del dolor en apertura y función mandibular. A la palpación presenta dolor en la zona de inserción del tendón del músculo temporal, el cual se elimina al anestesiar confirmando el diagnóstico de tendinitis del músculo temporal. Se realizan medidas primarias de manejo y luego se infiltra corticoides peritendineos. Al control el paciente no presenta dolor espontáneo ni en función. Conclusión: La tendinitis del músculo temporal es una patología frecuente, aunque subdiagnosticada. Debe realizarse el correcto diagnóstico diferencial con patologías frecuentes como dolor odontogénico, sinusitis, artralgia, dolor miofascial y migraña. El manejo dependerá del tipo de tendinitis. Habitualmente se presenta en conjunto con otros o dolores faciales, por lo que es importante conocer las distintas características clínicas de patologías con presentaciones similares.

ABSTRACT

El Histiocitoma fibroso maligno (HFM) fue descrito por Obrien y Stout en 1964 representando al 1-3 por ciento de los sarcomas del tejido blando de cabeza y cuello. Su histogénesis es incierta y el comportamiento biológico es propenso a la recidiva local y a las metástasis a distancia, preferentemente a ganglios linfáticos regionales, pulmón, hígado y huesos. Se presenta una paciente de 38 años de edad, con una lesión tumoral epicraneal recidivante, de consistencia blanda, fluctuante, no dolorosa, de aproximadamente 5 por 3 cm en región temporal derecha, se le realiza Tomografía computarizada de cráneo e Imagen por resonancia magnética, es intervenida quirúrgicamente resecándose totalmente una tumoración predominantemente quística en intima relación con el músculo temporal y reintervenida quince días después, el Rx de tórax, el ultrasonido abdominal y el survey óseo fueron negativos, el estudio histopatológico arrojó un Histiocitoma fibroso maligno pleomorfico por lo que es enviada al servicio de oncología para tratamiento adyuvante (radioterapia). Actualmente pasado un año se encuentra libre de enfermedad. Concluimos que el HFM primario del músculo temporal es una rara localización entre los sarcomas de partes blandas y aun más entre los de cabeza y cuello, donde el aumento de volumen no doloroso fue la forma clínica de presentación, la tomografía computarizada y principalmente las imágenes por resonancia magnética mostraron la relación músculo tumor, lográndose con el empleo de la cirugía y la radioterapia adyuvante alcanzar buenos resultados.

ABSTRACT

Lipoma within the musculature of the head and neck is rare. Most of them are infiltrating lipomas. There is no report of a well circumscribed lipoma of the temporalis muscle. We present a 58-year old man who had a non-pulsatile soft mobile mass in the right temporal fossa extending down in the cheek. The lesion was 15x7x3cm in size. Patient recovered well after surgery with a good cosmetic result

ABSTRACT

A total number of thirty albino mice were used in this study. The animals were taken at birth, seven, fourteen, twenty-one, thirty and sixty days after birth. Each age group comprised five animals. The temporal muscle was chosen as an example of mandibular elevator. The specimens were fixed in 4% neutral formalin and embedded in paraffin. ABSTRACT

O período de silêncio observado no eletromiograma dos músculos elevadores da mandíbula é resultante de uma inibiçäo reflexa do núcleo motor do nervo trigêmio induzida por estímulos provenientes dos fusos musculares e dos mecamorreceptores do periodonto. A partir da observaçäo de que a sua duraçäo estava aumentada nos registros eletromiográficos obtidos de pacientes com disfunçäo da ATM, o uso dessa medida passou a ser indicado como método objetivo de diagnóstico dessa disfunçäo. Entretanto, assim como alguns investigadores observaram na disfunçäo da ATM aumento significante na duraçäo do PS, outros colocaram dúvidas a respeito do uso por näo encontrarem esse aumento ou por näo o observarem em todos os pacientes com disfunçäo. Por outro lado, näo existem na literatura dados consistentes a respeito da duraçäo do PS eletromiográfico em outros tipos de disfunçöes oclusais e também näo foram encontrados relatos a respeito da duraçäo do PS eletromiográfico em pacientes portadores de fissura de lábio e de palato que apresentam grandes disfunçöes oclusais...

Fibromatosis of the temporal region is extremely rare. It has also been referred to as extra-abdominal desmoid which orininates from muscle or fascia and it is a highly recurrent tumor. This paper presents a case of temporal muscle fibromatosis and a brief review of the pertinent literature.
